Your GP can also refer you to us by fax (079 36 13 691) or you can e-mail your contact details to info@careforheart.nl. We will call you to schedule an appointment.

Cancelling or rescheduling an appointment
We consider it very important to be reliable to our patients. This means we must also be able to count on you. We therefore ask you to cancel appointments at least 24 hours in advance.

Any missed appointments without notice will result in you being charged € 43,30.
